Flagler Cares aspires to be a driving force behind bold innovations and community improvement creating a coordinated safety net of health and social care services.
Flagler Cares is a best practice organization whose purpose is to obtain and administer grant funding on a large scale, develop new programs to fill gaps in our safety net, and create a county-wide database and case-management system that will allow us to direct individuals to the services they need as well as to help agencies coordinate their services and communicate with each other.

Walk- in at Flagler County Village
Monday-Friday
8:30 AM - 4:30 PM
Intake Process:
Meet with one of Flagler Cares' Care Coordinators at the Flagler County Village for a brief screening to assess what needs you may have. After your screening, Care Coordinators will assist you with obtaining the services identified as needs.
